Five Circles Family Support Project
The main aims of the Five Circles Project is to reduce levels of isolation and exclusion experienced by children with learning disabilities and their families, by enhancing access to local communities, increasing skills and learning. The project will deliver a person centred assessment to determine the particular needs of each family and will develop a programme of activities to address those needs.
The project will complement school activities, provide training for parents, deliver sibling activities, and develop wider family support.
The project is to be delivered by Destined and 3 partners.
 Rossmar Special School
Rossmar is a co-educational Special School which caters for children with a wide range of learning difficulties, both moderate and severe and encompasses the age range of three to nineteen years. Admission is at any stage in a child's career where a need is identified.
Rossmar's role in the Five Circles project is to Provide referrals and input to person centred assessments for the young individuals. The Five circles project will work closely with the teachers at the child’s school to ensure activities outside school complement school programmes. Rossmar Special School will promote the project throughout the North West and will help to identify hard to reach beneficiaries. These children and young people will benefit from services as detailed in this project.
 Learmount Community Development Group
Learmount Community Development Group is a key community group in the area providing services to all sections of the community. The group is recognised for delivering quality programmes and supports to a large section of the North County Derry area.
The role of Learmount Community Development in the Five Circles Project is to provide referrals, programmes and sign posting. It is intended to base a number of the parent training programmes and the sibling activities in Learmount and draw upon their expertise in this area. The group will also promote the project to families living in rural areas. These families will benefit from services as detailed in this project.
 Western Health & Social Care Trust
The WHSCT has knowledge and understanding of children with learning disability and their families living in the area. Trust staff can provide support to children and young people who have been assessed as requiring specialist learning disability services while all other children and young people with disabilities are more appropriately supported within mainstream services.
The role of the WHSCT in the Five Circles Project is to promote the programmes to families throughout the North West, especially those living in rural and isolated areas and work with partner agencies to ensure increased support and opportunities for families.
